How can I add: “Suggest an improvement” ?

It seems that the options provided under "what brings you here?" do not include "Suggest an improvement." To add that option Here, you can consider reaching out to the relevant support team or website administrator directly. “What Brings You here” is set up within the Forms in Freshdesk.

First set up the ticket fields and then start choosing the fields you would like to display based on the form selected in Freshdesk. In your example, please navigate to Ticket fields -→ Add the choice “Suggest an improvement” in the Type field. Now Navigate to the Forms and create a form specifically for “Suggest an improvement”. Please choose the relevant fields from the ticket fields to show to your customer.
