At the moment it appears that the form templates are only available for Incidents and not for Major Incidents.
When Major Incidents occur, the idea is to try and get them logged as quick as possible so investigations can start and people can be advised of the ticket to link to. Having the ability to create templates for certain scenarios would be very useful. We have had our Major Incident Management team request this be set up so that they can try and streamline the processes. I am unsure as to why it is only available for Incidents and not Major Incidents.
