As the title says, I would like to see a multi select field type added.
We have use cases where a requester would need to select several answers from the one question.

We desperately need this. The only alternative is to use a long list of check boxes that we can't even group together.
This is a much needed feature that would really help with selecting multiple affected applications/services. As Brandon mentioned, you could achieve this with check boxes but they can't be grouped together tightly so they make the form too large if you have 4-5 boxes per 'field'. You also can't have 'headings' in the form layout which means that each individual check box has to be labeled thoroughly so it is clear that they are related.
