Why is this page being generated when a New Ticket is being generated?

Is this a limitation o the Free level? This was working for us and now this is all we see.
This is bad Karma and the fact that it takes you days to respond to a ticket is just crazy. How can we trust an organization that wants us to improve our own Customer Service when we receive such lousy service.
