Is it possible to create a service catalog item that a requestor can use to select items from a list and add a quantity to a “request form” then have the total cost of everything being requested shown on the form before it get’s submitted?

Yes, it is indeed possible to display the total quantity of requested items and their cost while a requester is submitting a request. These details will be shown on the form before the request is submitted. Please enable the "Show Quantity" option, add the amount, and check the "Visible in Portal" option for the service catalog item.
