I have created this service item and need to get these two custom fields inserted into the approval mail.
The notification email. Trying to find the custom fields from the service item under Insert Placeholders:

Only option you have are this, All fields and All visible fields
You can use API to send approval, there you can customize the mssage as you want but it comes with its own caveats.
