How to trigger approval email to a line manager from a name within a field in a service item?

Hi, 

I used to have a workflow that triggered an email to the line manager of the name of a person that was selected from a drop down option within a service item. See example below.

(The approval would go to the line manager of the person selected in the 2nd drop down field and not the original requester)

The workflow now no longer works and I am unable to fix it - is anyone able to advise how I can set his up again?
