Alert workflow not triggered to create new incident

I am trying to automatically raise an incident ticket when an alert is raised.

 

I have a simple workflow to attempt this but it isn’t bring triggered. Help appreciated.

 

 

 

New alerts are generated, however no incident ticket is raised. The “Activities” section of the alert has no log of the trigger, and the Alerts Workflow Automator section is missing an “Execution log”, so feel a bit lost.
